I provide initial consultations that are either a one-time evaluation to provide a plan of care for the client or family/attorney-in-fact or this becomes the beginning of a professional relationship to then assist as a care manager. The initial consultation includes assessments of psychosocial, cognitive, and functional status. I explore financial needs, mental health and physical concerns, support system involvement and availability, and more.
This is a person-centered approach to managing the everyday concerns that occur in later life with coaching, advising, advocacy, and navigating with creative problem solving. I help answer the "what now" as life evolves and ease the stress of caring for a loved one.
I attend health care appointments, facilitating communication between the client, provider, and family. I reiterate education from these appointments as needed.
I keep family members and professionals informed of the well-being and changing needs of the client.
I provide opportunities for engaging in social, recreational, or cultural activities that enrich life.
I refer to or consult with legal specialists.
I review or oversee bill paying or consult with accountants or attorneys-in-fact for finance.
I provide local, cost-effective resource referrals.
I monitor home care, observing changes or potential risks of exploitation or abuse and protecting the vulnerable person.
I assist with housing, transitional, and relocation services, helping clients and their families select the appropriate residential option and make all the needed arrangements to make the transition as smooth as possible.

Unfortunately, Medicare and other insurance companies do not cover care consultation or management services. Services are billed privately by the hour.
